REGISTRATION

For online registration:
*A copy of both sides of the NRIC, GIF/JPEG format and under 10 MB in size, needs to be submitted.
* There will be admin and credit card charges incurred for the use of online registration.
* For participants under the age of 21, please upload a copy of the indemnity form during registration.
Submission of Registration Forms:
Each team is required to submit only 1 Registration Form per team.
For Direct Submission of Registration Forms:
1) Submission of forms can be done directly at our booths in Nanyang Technological University (NTU) with the filled Registration Form and cash or cheque.
2) Booth dates and times will be updated on the website again.
For Mailing of Registration Forms:
1) Payments should be made by cheque ONLY and made payable to "NTU Student Union".
2) Kindly indicate at the back of your cheque:
- X-Physique 2012
- The category you are participating in
- Your Team Name
*3) Students participating in the JC Shield or Inter-Varsity Challenge MUST send in a copy of their NRIC and student pass or matriculation card.
4) Things to include:
- X-Physique 2012 Registration Form (1 for entire team)
- Cheque
- Photocopy of NRIC and student pass/matriculation card (students)

5) Successful applicants will be notified via a confirmation email or call within 2 weeks upon submission of registration forms.
